Forms Policy

We will make every effort to get school, camp, athletic, and other forms filled out as quickly as possible. It is our office policy to get forms done within five working days. We charge a $5.00 processing fee for all forms, except for medication authorization forms for school. If you bring your forms in at the time of your child's well check, the fee will be waived.
